celebration at the Medicare Information & Vitality Center this Monday,
October 15, from 10:30 a.m. to 4:30 p.m.
Discover what the center can do for you: Medicare information, community resources, health
education, social events and more. Refreshments will be provided along with
hourly raffles. 2013 enrollment applications will also be available.
The Medicate
Information & Vitality Center is located at the Pomona Valley Health Center
Building, 1601 Monte Vista Avenue, Suite 275 in Claremont. RSVPs are required as
